q                            Team Competition Conduct Form signed by all required

q                            Medical Release Form for each student

q                            Special Needs Form (if applicable)

q                            Photo of school logo (if this is your first competition—we will

use this to make your school flag.)

We need to make all the arrangements necessary to have a terrific decathlon; therefore, if you are unable to send us your information on time, your team will not be allowed to participate. The Competition Manager requires sufficient time in order to evaluate each student’s transcripts for all teams. 

ONE LAST REMINDER- TRANSCRIPTS MUST BE INCLUDED WITH THE REGISTRATION FORM.
